The Secondary School Report is a required component of the application for most U.S. universities. NACAC recommends that the student completes Section I and then delivers the form to their school counselor, who will provide an overview of the applicant’s academic record in Section II and Section III.

This form can be submitted directly to the universities to which you are applying or via your application portal (if applicable). The Secondary School Report is sometimes confused with the school profile, which provides summary information about the high school an applicant attends.

This PDF enables the student and the counselor to complete the form onscreen and can be emailed.
